The failure is nearly always the record, not the conversation.
Nothing is written down until review week, so the year collapses into recency bias.
Notes are typed or dictated as things happen and stamped with the date they occurred.
A rating becomes the conversation. The work, the context and the fix get lost.
No scores at all. Feedback is grouped into themes drawn from the notes themselves.
Objectives are pasted in and never acknowledged, so nobody can prove what was agreed.
Each objective is accepted or declined by the employee, with a reason on the record.

No 1–5 scores, no forced distribution, no stack ranking. Ratings create arguments about the number instead of the work.
Private notes stay private
A manager's private note is never shared with the employee and never flows into a plan, a letter or a mediation room.
